1095-C Employer-Provided Health Insurance Offer and Coverage

You can now choose to receive the Form 1095-C electronically rather than in the mail. The University is required to provide the form for your tax preparation, but it’s your decision how to receive it.

Find instructions for electing to receive your 1095-C electronically here. If you do not elect to receive your 1095-C through Workday, your form will be mailed to your home address by March 2, 2022.
