Committee Forms

These forms should be completed in a word processing program and emailed as PDF file to the CRT Administrative Support person. Hard copies of the supervisor evaluation form, with signatures, should be dropped off at the Dean’s Office in Loyola Hall 17. The forms also include the instructions for supervisors.

Self-Evaluation Form - To be completed yearly by tenure-track faculty members and non-tenure-track, ranked faculty members in their first five years. To be completed at least every five years by tenured faculty members and non-tenure-track, ranked faculty members after their first five years.

Ranked Faculty Member Review and Deadlines

Faculty member self-evaluation provides the first evaluation. Yearly for ranked faculty members before tenure and/or promotion (or in the first five years for ranked non-tenure track faculty members). Every five years for other ranked faculty members.

  • Due to supervisor by Sept. 21 if tenure track, applying for promotion and/or tenure, or a ranked non-tenure track faculty member eligible to seek tenure/tenure track.
  • Due to supervisor by Oct. 15 if tenured and undergoing evaluation, a ranked non-tenure track faculty member, or a term faculty member undergoing review.

Supervisor evaluation (or Special Academic Evaluator) considers the self-evaluation, the required course observation(s), and any other evidence, and produces and independent evaluation and recommendation.

  • Due to CRT by Oct. 7 if faculty member is tenure track, applying for promotion and/or tenure, or a ranked non-tenure track faculty member eligible to seek tenure/tenure track.
  • Due to CRT by Oct. 31 if faculty member is tenured undergoing evaluation, a ranked non-tenure track faculty member after the first five years, or a Term faculty member undergoing review.

Committee on Rank & Tenure evaluation considers the self-evaluation, the supervisor evaluation, and any other evidence, and produces an independent evaluation and recommendation. After the first year, after the third year, for promotion/tenure applications, and for post-tenure reviews (every 5 years).

  • Due to Dean and Provost by Dec. 1 if faculty member is in second tenure-track year.
  • Due to Dean and Provost by Jan. 31 if faculty member is tenure track (not in second year), applying for promotion and/or tenure, or a ranked non-tenure track faculty member eligible to seek tenure/tenure track.
  • Due to Dean and Provost by Feb. 28 for all other faculty-member evaluations.

The Dean considers the self-evaluation, the supervisor evaluation, the Committee on Rank & Tenure Evaluation, and any other evidence, and provides an independent evaluation and recommendation after the faculty member’s first year and for promotion and/or tenure applications. The Dean also receives the Committee on Rank & Tenure evaluation in other years whenever one is provided.

  • Due to Provost, supervisor, the faculty member, and the CRT by Dec. 15 if faculty member is in second tenure-track year.
  • Due to Provost, supervisor, the faculty member, and the CRT by Feb. 15 if faculty member is in second tenure-track year.

The Provost considers the self-evaluation, the supervisor evaluation, the Committee on Rank & Tenure Evaluation, the Dean Evaluation, and any other evidence, and provides a recommendation to the President for promotion and/or tenure applications. The Provost also receives the Committee on Rank & Tenure and Dean evaluation after faculty members’ first years.

No one can serve in two roles in the process.


Faculty Deadlines

These deadlines are very important to uphold in order to keep the CRT process moving, and late action by a supervisor could even lead to a grievance. The CRT needs time to advocate for faculty at the administrative level. If a short extension of time is needed, the supervisor needs to communicate the situation to the committee.

Sept. 21 Self-evals for all tenure-track faculty members, tenure and promotion applications due to supervisor (also sabbatical applications due to supervisor).
Oct. 7

Chair must have observed teaching, met with faculty member, and written supervisor eval letter; all supervisor letters (for tenure and promotion applications, tenure-track self-evals, and sabbatical requests) due to the CRT (via Dean’s Office) or FDC (for sabbatical applications).

Oct. 15 Any clarifying comments that a faculty member who submitted materials by Sept. 21 wants to submit due (this is uncommon).

Self-evals for post-tenure reviews due to supervisor; also any STEPP requests not covered by other applications/self-evals; also self-evals of continuing term faculty.

Oct. 31

Chair must have observed teaching, met with faculty member, and written supervisor eval letter; submit supervisor letter due to the CRT (via Dean’s Office).

Nov. 7 Any clarifying comments that a faculty member who submitted materials by Oct. 15 wants to submit due (this is uncommon).
